College fee and department fee receipts (printed from steps 2 and 3)
Caleb University and JAMB admission letters (JAMB admission letter must be coloured)
WAEC or NECO/GCE results
JAMB result
Birth certificate
One passport photograph

Where is the COCIS Office?

In the Admin block. It's on the first floor if you're entering from the printing shop, or on the same floor as the reception if you're entering from there.
